Evaluation of Primary Care Management in Phibun Mangsahan District Health Network

Abstract

     This Research and Development (R&D) aimed to 1) study and analyze the context, problems, and obstacles of proactive pharmacy services and consumer protection across three primary care models: Primary Care Units (PCUs), Network Primary Care Units (NPCUs), and Sub-district Health Promoting Hospitals (SHPHs); 2) design and develop a proactive drug problem and consumer protection management system; and 3) evaluate and compare pharmaceutical outcomes among the three models. The sample consisted of representatives from the three primary care models and 90 chronic disease patients taking five or more medications (30 per model), selected through purposive sampling. Research instruments included Standard Operating Procedures (SOPs) for home visits, the AppSheet application integrated with Google Workspace, and low-tech patient tools. Data were analyzed using frequency, percentage, mean, standard deviation, One-way ANOVA, and Chi-square test at a significance level of 0.05.

     The findings revealed that the proactive drug management system effectively bridged operational gaps among the three primary care models, enabling systematic identification and resolution of drug-related problems (DRPs) and risky health products in the community. This successfully achieved the target resolution rates and minimized recurrent adverse drug events. These empirical findings provide valuable insights for administrators to optimize workforce allocation, home visit frequencies, and primary care pharmacy policies to ensure maximum patient safety.
