Abstract

This research and development study aimed to investigate the existing situation, develop, and evaluate a seamless community-based care model for psychiatric patients at high risk of violence (SMI-V) through the integration of the LINE Official Account (LINE OA) “Pha Ya Ban Klai Jai” and collaboration with community network partners at Phayakkhaphum Phisai Hospital, Maha Sarakham Province, Thailand. The study was conducted from October 2023 to September 2024. A total of 180 participants were purposively selected, comprising 60 SMI-V patients, 60 primary caregivers, and 60 healthcare personnel and community network partners. The research process consisted of three phases: (1) situational analysis and preparation, (2) model development, and (3) implementation/review and evaluation. Research instruments included a medication adherence assessment, the Overt Aggression Scale (OAS), a caregiver knowledge questionnaire, a network participation assessment, and a satisfaction questionnaire. Data were analyzed using descriptive statistics and statistical tests comparing pre- and post-intervention outcomes.

The findings revealed that, following implementation of the developed model, the mean medication adherence score significantly increased from 58.30 ± 7.20 to 70.10 ± 5.80, while the mean aggression score (OAS) significantly decreased from 1.45 ± 0.68 to 0.62 ± 0.49 (p < .001). Caregiver knowledge increased from 52.0% to 85.0%, and the mean network participation score increased from 58.40 ± 8.60 to 72.80 ± 6.90 (p < .001). Furthermore, the medication nonadherence rate decreased from 37.6% to 18.2%, while 80.0% of patients received continuous appointment and follow-up care. Additionally, 92.0% of caregivers were able to report emergencies and coordinate timely assistance. Overall satisfaction with the developed model was rated at the highest level (4.55 ± 0.38).
