This research and development study aimed to examine the situation and factors related to treatment retention among patients with drug use problems, to develop a treatment model promoting one-year retention, and to evaluate the model at Nadun Hospital, Maha Sarakham Province, Thailand. The study comprised four phases following the action research cycle. The model was developed with a 16-member multidisciplinary team, staff of sub-district health promoting hospitals, community leaders, and village health volunteers. The evaluation sample comprised 139 new outpatients admitted between October 2023 and September 2024, compared with a historical group of 71 patients treated in fiscal year 2023. Data were analyzed using descriptive statistics, the chi-square test, and thematic analysis. Dropout was related to patient, family, provider, and service system factors.
The resulting NADUN Model consists of five components: Need-based assessment, Alliance building, Distance-free follow-up, Unity of family and community, and Navigation by a case manager. After implementation, the one-year retention rate rose from 84.51% to 90.65%, a difference of 6.14 percentage points (95% CI: −3.57 to 15.85), which was not statistically significant (χ² = 1.75, p = .186). Satisfaction with the model among patients and the multidisciplinary team was at a high level (M = 4.46, SD = 0.52 and M = 4.38, SD = 0.50, respectively).
