Abstract

The objectives of this quasi experimental research by two group design were to 1) study the nursing situation of psychiatric patients with violent behavior, 2) develop nursing practice guidelines for psychiatric patients with violent behavior, and 3) evaluate the outcomes of implementing these guidelines in the Emergency Department of Pho Thong Hospital, Ang Thong Province. The research process was conducted based on the National Health and Medical Research Council (NHMRC, 1998) clinical practice guideline development framework, divided into three phases: Phase 1 Guideline Development, which involved focus group discussions with professional nurses and nurse aides, a literature search for evidence-based data, and quality appraisal using the AGREE II tool (yielding a score of 0.84); Phase 2 Guideline Implementation and Refinement, where the guidelines were trialed and revised through two cycles with professional nurses until finalized (with expert quality appraisal yielding an AGREE II score of 0.91); and Phase 3 Evaluation of Guideline Implementation, involving a sample group of 32 psychiatric patients with violent behavior (divided equally into 16 in the control group and 16 in the experimental group). Data were collected from December 1, 2025, to June 30, 2026, and analyzed using descriptive statistics, Chi-square, and t-test.

Results of the Research. The mean scores for personnel knowledge and skills in managing violence after implementing the guidelines were significantly higher than before implementation at the level of < 0.005. The overall opinion of the emergency department personnel regarding the feasibility of implementing the guidelines was at a high level. The incidence of recurrent violence in the experimental group was lower than that in the control group. Personnel injuries in the experimental group were significantly lower than those in the control group at the level of < 0.005. The overall satisfaction of personnel toward the nursing practice guidelines was at the highest level ( ). The overall satisfaction of patients and caregivers toward the nursing practice guidelines was at the highest level ( ).
