Development of Home-Based Nursing Care Guidelines for Stroke Patients Toward Continuous and Sustainable Care According to the Primary Care Nurse Role, Phukieo Chalermphrakiat Hospital, Chaiyaphum Province.

This Research and Development (R&D) study aimed to develop and evaluate the effectiveness of home-based nursing care guidelines for stroke patients toward continuous and sustainable care according to the primary care nurse role at Phukieo Chalermphrakiat Hospital, Chaiyaphum Province, guided by the Transitional Care Model. The study was conducted from October 2023 to September 2024 (12 months) in four phases. The research instruments comprised the nursing care guidelines, an appropriateness evaluation form, a knowledge test and a skill checklist for nurses, the Barthel ADL Index, the PHQ-9, the EQ-5D-5L, a patient outcome record form, and satisfaction questionnaires. Data were analyzed using descriptive statistics, paired t-test, Wilcoxon signed-rank test, and chi-square test.
Results: Five care-transition gaps were identified: discharge planning, primary care nurse competency, continuity of home visits, information transfer, and caregiver readiness. The developed guidelines comprised five components and a monthly PDCA quality-monitoring mechanism. After implementation, the nurses' knowledge scores increased from 11.67 ± 2.99 to 16.87 ± 3.09 and skill scores from 18.53 ± 5.49 to 26.60 ± 6.48 (p<.001). Patients' Barthel ADL Index scores increased from 11.57 ± 2.84 to 15.50 ± 3.20, PHQ-9 scores decreased from 6.73 ± 4.12 to 4.50 ± 3.93, systolic blood pressure decreased from 153.20 ± 13.81 to 140.03 ± 15.97 mmHg, and EQ-VAS scores increased from 59.60 ± 7.51 to 72.90 ± 10.61 (all p<.001). Physical complications decreased from 25.00% to 6.67% (p=.028), and the proportion of patients receiving home visits increased from 45.00% to 100.00% (p<.001). No recurrent stroke or hospital readmission occurred during follow-up. Satisfaction among patients/caregivers (4.24 ± 0.43) and nurses (4.32 ± 0.44) was at a high level.
