Effects of Case Management Program on Uncontrolled Diabetes Mellitus Patients

Abstract

A quasi-experimental group was used to evaluate the effects of a case management
program in uncontrolled diabetes mellitus patients. Thirty-eight samples drawing by simple
random sampling from diabetes patients with HbA1c more than 7% and served in Bansuan
Health Promoting Hospital total of 72 people. The sample size is calculated using the
compare mean formula. Duration this research is 12 weeks. Case management program
consists of activities. 1)A multidisciplinary team was created by the researchers as case managers 2) Development of patient care plan 3) Patient care according to patient care plan
4) Team meetings to adjust patient care plans to suit individual problems 5) Behavior
modification 6) Supported by home visits 7) Motivation to change behaviors such as
appreciation. Collected self-care behavior data. And HbA1c for compared before and after
the case management program. Data were analyzed by means, standard deviation and
paired t-test.
The results were as follow: 1) Self-care behaviors of uncontrolled diabetes mellitus
patients before entering the case management program the average score was 3.22 (S.D. =
0.19). After the case management program 12-week mean score of self-care behavior
increased to 3.35 (S.D. = 0.15), which was significantly higher (p-value <.05) 2) The
HbA1c before entering the case management program was 8.97%. After the case
management program HbA1c were reduced to 7.39%, significantly lower. (p-value <.05)
