The Research and Development aimed to study the effect of developing self-management programs to reduce the risk of cardiovascular disease in people with diabetes and high blood pressure. The sample consisted of diabetic and high blood pressure patients who were assessed for cardiovascular risk score (CVD Risk Score) in 2020 and received services in a chronic disease clinic. Renu Nakhon Hospital The instrument used in this operation was divided into 2 parts. Part 1 The study instrument consisted of self-management program to reduce the risk of cardiovascular disease in diabetic patients and Hypertension Part 2 The tools used for data collection were general information questionnaire. food consumption behavior assessment form exercise Medication and mood management Clinical outcomes and risk of cardiovascular disease in the next 10 years. Analysis of general data. using percentage values, food consumption health behaviors exercise Drug use and mood management used numbers, percentages, and mean values. and compare the clinical outcomes before and after Join the program using the Pair t-test statistics.
The results Showed that The sample group before and after participating in a self-management program to reduce the risk of cardiovascular disease, waist circumference, BMI. Blood Pressure Level, HbA1c), cholesterol and the risk of cardiovascular disease in the next 10 years (CVD Risk Score) was significantly different at the .05 level.
