Nursing care of Ischemic Stroke patients in Emergency room : Case study

The objective of this study is to guide nursing care of Ischemic Stroke patient who receives anticoagulants and as a guideline for improving the quality of nursing for acute stroke patients. Selecting a stroke patient admitted to the emergency room and ward of Phimai Hospital, Nakhon Ratchasima during October 2021 – March 2022
Results: Case Study Thai female patient, 56 years old Major Symptoms: Facial palsy, slurred speech and left limb weakness happened 1 hour before coming to the hospital. Patient was non-adherence to diabetes and hypertension medications for 1 year. Doctor diagnosed the disease as acute cerebral ischemia. Patient was treated with anticoagulant medicine type rt-PA. Before receiving the rt-PA, patient had hypertension and hyperglycemia, received antihypertensive and hypoglycemic medicine. After receiving rt-PA drug, she was detected no complications no cerebral hemorrhage. Patient also had swallowing problems and weakness .Received treatment from observe neuro sign and rehabilitation treated by physiotherapist team. The patient's symptoms improved. Doctor discharged patient to home after 3 days hospital stay. Final diagnosis was Acute Cerebral infraction at right frontal lobe with Hypertension Emergency with Type 2 Diabetic mellitus
