ABSTRACT
This quasi-experimental study aimed to examine the effectiveness of a perioperative nursing program among orthopedic and general surgical patients undergoing general anesthesia at Sawankhalok Hospital, Sukhothai Province. The sample consisted of 60 patients scheduled for elective surgery, divided into an experimental group and a control group of 30 each. The experimental group received a nursing program based on the Enhanced Recovery After Surgery (ERAS) concept and Kolcaba’s Comfort Theory, comprising four components: preanesthetic risk assessment and preparation, preoperative education and anxiety reduction, post-anesthesia care unit (PACU) care, and systematic surveillance of postoperative complications during the first 72 hours. The control group received usual nursing care. Data were collected using the Thai version of the State-Trait Anxiety Inventory (STAI), the Numerical Rating Scale (NRS 0–10), a postoperative complication surveillance record, and a satisfaction with nursing care questionnaire. Data were analyzed using descriptive statistics, paired t-test, independent t-test, chi-square test, mann-whitney U test and fisher's exact test.
The results revealed that, after the intervention, the experimental group’s mean preoperative anxiety score decreased from 61.60 ± 4.53 to 34.73 ± 4.80, and the mean satisfaction score increased from 55.93 ± 4.55 to 87.37 ± 6.28 (p < 0.001). Compared with the control group, the experimental group had significantly lower anxiety and higher satisfaction (p < 0.001). Pain scores of the experimental group were significantly lower than those of the control group at 24 hours (3.87 ± 0.82 vs. 6.93 ± 0.69) and 48 hours (2.50 ± 0.90 vs. 6.40 ± 0.89) (p < 0.001). No postoperative complications occurred in the experimental group (0.0%), compared with 26.7% in the control group (p = 0.005). The program is effective in reducing preoperative anxiety, postoperative pain, and complications, and in improving patient satisfaction. Community hospitals should adopt it as a standard practice for elective surgical patients undergoing general anesthesia
