An Investigation of the First Case of Multidrug-Resistant Tuberculosis in Kaibangrachan District, Singburi Province, Thailand, 2023

Abstract

The investigation of the first case of Multidrug-Resistant Tuberculosis (MDR-TB) in Khai Bang Rachan district, Sing Buri province, in 2023 (B.E. 2566) aimed to 1) confirm the diagnosis, describe the clinical characteristics of the patient, identify potential sources of infection and drug resistance in the Mycobacterium tuberculosis strain, investigate contacts for latent or active TB, and implement appropriate control measures. A descriptive epidemiological investigation was conducted between May and June 2023. This included reviewing hospital medical records and utilizing the National Tuberculosis Information Platform (NTIP) database. The patient and relatives were interviewed to identify household and close contacts defined by specific criteria. Diagnostic confirmation involved chest X-ray, Interferon-Gamma Release Assay (IGRA), and sputum testing via the Xpert MTB/RIF method, complemented by an environmental assessment of the patient's residence. The results revealed that the patient was a Thai man, 62 years old with underlying gout and stage four chronic kidney disease. The patient had no history of prior TB diagnosis or treatment. In February 2023, the patient presented with a dry cough and weight loss. Sputum microscopy revealed acid-fast bacilli (AFB 2+). A line probe assay conducted in March 2023 identified rifampicin resistance, leading to a diagnosis of MDR-TB. This diagnosis was subsequently confirmed in April 2023. Among household contacts, no additional active TB cases were found, but there was case of latent TB infection (LTBI). The patient’s home was well-ventilated, with adequate sunlight, the use of a separate bathroom, and proper sputum disposal practices. Conclusion: This case was identified as primary MDR-TB. Furthermore, the patient exhibited several significant risk factors associated with the development of drug-resistant tuberculosis, including smoking, alcohol use, substance abuse, stage four chronic kidney disease (CKD), and being underweight.
