Effects of Discharge Planning Program in the uncontrolled type 2 diabetes patients at Charoensin Hospital, Sakon Nakhon Province.

This quasi-experimental research aimed to study the effects of discharge planning program in the uncontrolled type 2 diabetes patients at Charoensin Hospital, Sakon Nakhon Province. The sample was a patient who admitted at inpatient department of Charoensin hospital with hyperglycemia or hypoglycemia during the month of May to July 2023. Purposive sampling was used for selecting the sample totally of 30 participants. Data collection consists 3 questionnaires; characteristic of sample, the knowledge of diabetic, and the behavior of self-care. The experimental instrument was the discharge planning program. Data were analyzed by descriptive statistic and paired t-test statistics.
The result found that, Diabetes type 2 patients after receiving a discharge planning program, there was a statistically significant increase in diabetes knowledge than before the program (t=-10.638, p=<.001). And increasing score of self-care behavior after the program (t=-10.733, p=<.001), significantly at .05 level. On the other hand, there was decrease fasting plasma glucose levels after receiving program (t=6.586, p=<.001), respectively.
