Development of a model of continuation of care after critical to mid-term in patients with acute ischemic stroke

This study was research and development aimed to develop and study the effects of a post-critical continuation of care model in patients with acute ischemic stroke. Ban Dung Crown Prince Hospital Udon Thani Province. The research was conducted between October 2021 - September 2022. The samples 32 ischemic stroke patients by purposive sampling. Data were collected by assessment form for the ability to perform daily activities. disability assessment form a satisfaction questionnaire for multidisciplinary personnel in the form of group discussion recordings Quantitative data were analyzed by frequency, percentage, mean, standard deviation, t-test, and qualitative data were analyzed by content analysis.
The results showed that the model consisted of 1) a multidisciplinary team 2) a care plan, a model of care for stroke patients that consisted of 6 processes, namely 1. Access and entry (Access & Entry) 2. Patient Assessment 3. Planning of Care 4. Discharge Planning 5. Information and Empowerment for Patients/ Families) 6. Continuity of Care and 3) Clinical Practice Guidelines Collaboration of relevant multidisciplinary teams can be practical in a hospital context. resulting in the majority of care outcomes having a positive change The mean ability to perform daily activities was statistically significantly higher than the group before the model development (p = .00). And 2 weeks after discharge from the hospital, cerebrovascular patients had less severe and abnormal level of disability than the date of admission to the hospital. Satisfaction with the care model found that the overall level of satisfaction was at a high level.
