Effects of a Home Visitation Program Combined with Self-management Support on Clinical Outcomes of Elderly Patients with Type 2 Diabetes.

This quasi-experimental research, employing a one-group pretest-posttest design, aimed to investigate the effects of a home visitation program integrated with self-management support on clinical outcomes and health behaviors among elderly patients with uncontrolled type 2 diabetes (HbA1c ≥ 7.5%) in Sawang Daen Din sub-district. A purposive sample of 41 participants was selected. The 13-week intervention consisted of health education and three proactive home visits based on Kanfer & Gaelick-Buys' theory. Data were analyzed using descriptive statistics and paired sample t-tests.
The results revealed that after participating in the program, the participants' clinical outcomes significantly improved: the mean HbA1c level decreased from 7.96% to 7.77% (p=.014). Both systolic (SBP) and diastolic blood pressure (DBP) significantly declined (p<.001). Body mass index (BMI) decreased slightly but without statistical significance (p=.162). Regarding health behaviors—including diet, exercise, treatment follow-up, and stress management—the mean scores for both overall and all sub-dimensions significantly increased (p<.001). In conclusion, this program is effective in enhancing both clinical outcomes and health behaviors for elderly diabetic patients in the community.
