Abstract

Liver fluke infection is an important risk factor for cholangiocarcinoma and remains a public health problem in Khukhan District, Sisaket Province. This two-group pretest–posttest quasi-experimental study aimed to compare health literacy, community participation, and liver fluke prevention behaviors within and between groups. The sample comprised 100 at-risk individuals, divided into an experimental group from Sri Trakul Subdistrict and a comparison group from Sadao Yai Subdistrict, with 50 participants in each group. Participants were selected using two-stage random sampling. The experimental group received an eight-week program integrating Nutbeam’s health literacy concept with community participation, whereas the comparison group received routine health education. Data were collected using questionnaires and analyzed using descriptive statistics and multivariable linear regression.

The results showed that, after the intervention, the experimental group had significantly higher mean scores for all six components of health literacy, community participation, and liver fluke prevention behaviors than before the intervention (p < 0.001). The prevention behavior score increased by 6.56 points (adjusted mean difference = 6.56; 95% CI: 5.34–7.78). Post-intervention comparisons between the groups showed that the experimental group had significantly higher scores for all outcomes than the comparison group (p < 0.05). The adjusted mean differences between the experimental and comparison groups were 1.22 points for knowledge, 3.41 points for access to health information, 2.24 points for health communication, 1.24 points for decision-making, 1.82 points for media literacy, 2.81 points for self-management, and 4.14 points for community participation. The greatest between-group difference was observed in prevention behaviors, with an adjusted mean difference of 5.82 points (95% CI: 4.64–7.01). The program was effective in improving health literacy and liver fluke prevention behaviors and can be scaled up to other high-risk areas by utilizing village health volunteers and community participation mechanisms.
