บทคัดย่อ

บทนำ: องค์การอนามัยโลก รายงานว่าประเทศไทยเป็นหนึ่งในประเทศที่มีอัตราการเสียชีวิตจากอุบัติเหตุทางถนนในระดับสูงเมื่อเปรียบเทียบในระดับโลก ดังนั้น การทำความเข้าใจสาเหตุและปัจจัยที่เกี่ยวข้องกับการเกิดอุบัติเหตุทางถนนจึงมีความสำคัญ เนื่องจากสามารถใช้เป็นข้อมูลเชิงประจักษ์เพื่อสนับสนุนการกำหนดนโยบาย การออกมาตรการด้านความปลอดภัยทางถนน และการบังคับใช้กฎหมายอย่างมีประสิทธิภาพ อันจะนำไปสู่การลดความสูญเสียจากอุบัติเหตุในอนาคต

วัตถุประสงค์: เพื่อประเมินความรุนแรง และปัจจัยที่เกี่ยวข้องกับการเกิดอุบัติเหตุในรถที่มีปริมาตรความจุของกระบอกสูบที่แตกต่างกัน รวมถึงปัจจัยที่มีความสัมพันธ์กับการเกิดอุบัติเหตุ

วิธีการศึกษา: การวิจัยเชิงพรรณนา (Cross-Sectional Study) โดยใช้ข้อมูลผู้บาดเจ็บจากการอุบัติเหตุทางถนนโดยใช้รถจักรยานยนต์เป็นพาหนะหลักในการเดินทาง ในจังหวัดภูเก็ต ในปี พ.ศ. 2564 – 2566

ผลการศึกษา: ปริมาตรกระบอกสูบมากกว่า 150 cc การไม่สวมหมวกกันน็อค ความเร็วในการขับขี่มากกว่า 90 กม./ชม. มีความสัมพันธ์กับการมี AIS score สูงขึ้นอย่างมีนัยสำคัญทางสถิติ โดยเฉพาะการไม่สวมหมวกกันน็อคซึ่งมีความเสี่ยงสูงที่สุด การไม่สวมหมวกกันน็อค มีความสัมพันธ์กับการเกิดอุบัติเหตุอย่างมีนัยสำคัญทางสถิติ (p < 0.001) นอกจากนี้ ความเร็วในการขับขี่ตั้งแต่ 90 กม./ชม. ขึ้นไป และ การขับขี่บนถนนหลัก ก็มีความสัมพันธ์กับการเกิดอุบัติเหตุอย่างมีนัยสำคัญเช่นกัน (p = 0.02 และ p = 0.01 ตามลำดับ) ในด้านความรุนแรงของการบาดเจ็บ โดยความรุนแรงของอุบัติเหตุมีความสัมพันธ์เชิงบวกกับค่า ISS อย่างมีนัยสำคัญทางสถิติ (p < 0.01) โดยพบว่าทุกๆ ระดับความรุนแรงของอุบัติเหตุที่เพิ่มขึ้น ส่งผลให้ค่า ISS เฉลี่ยสูงขึ้น 1 คะแนน

สรุปผลการศึกษา: ผลการศึกษาชี้ให้เห็นว่า การไม่สวมหมวกกันน็อค และการขับขี่ด้วยความเร็วสูง ประกอบกับปัจจัยเฉพาะด้าน เช่น สภาพยางรถที่ไม่สมบูรณ์และแสงสว่างบนถนนที่ไม่เพียงพอ เป็นปัจจัยสำคัญที่สัมพันธ์โดยตรงกับความรุนแรงของการบาดเจ็บ (AIS score) สิ่งที่ทำให้การศึกษานี้โดดเด่นและแตกต่างจากงานวิจัยในอดีต คือการวิเคราะห์ร่วมกับปัจจัยด้านสภาพแวดล้อมเฉพาะถิ่น (Local environment) ซึ่งพบว่ามีนัยสำคัญ ดังนั้นมาตรการเชิงรุกที่ควบคู่ไปกับการปรับปรุงสภาพทางกายภาพของถนนจึงเป็นแนวทางที่ช่วยลดอุบัติเหตุได้ตรงจุดกว่าการรณรงค์เพียงอย่างเดียว
