This study aimed to evaluate the use of the INHOMESSS assessment in palliative care patients after hospital discharge, compare quality of life before and after its use, and assess patient and family satisfaction. A pre-experimental one-group pretest-posttest design was conducted among 35 palliative care patients discharged from Chumphon Khet Udomsak Hospital. Participants were selected by purposive sampling and followed for four weeks. The instruments included a demographic questionnaire, the INHOMESSS assessment, a quality-of-life questionnaire, and a satisfaction questionnaire. Data were analyzed using frequency, percentage, mean, standard deviation, and paired samples t-test at a significance level of .05.
After implementation, the proportion of patients with normal nutritional status increased from 71.43% to 74.29%, those with clean and orderly home environments increased from 94.29% to 100.00%, and those with correct medication use increased from 82.86% to 88.57%. Mean quality-of-life scores before and after the INHOMESSS assessment were 2.66 (SD = 0.18) and 2.64 (SD = 0.21), respectively, with no statistically significant difference (t = 1.29, p = .205). Overall satisfaction was high (M = 4.20, SD = 0.12). These findings suggest that INHOMESSS provides a structured framework for comprehensive patient assessment and supports continuity of palliative care from hospital to community, although no significant improvement in quality of life was demonstrated.
