บทคัดย่อ
การวิจัยแบบกลุ่มย้อนหลัง (Retrospective Cohort Study) มีวัตถุประสงค์เพื่อเปรียบเทียบการเกิดภาวะเลือดออกในทางเดินอาหารและภาวะการย่อยอาหารผิดปกติ ระหว่างผู้ป่วยที่ได้รับอาหารทางสายยางภายในและภายหลัง 24 ชั่วโมง หลังใส่ท่อช่วยหายใจ และศึกษาปัจจัยที่เกี่ยวข้อง ทำการศึกษาข้อมูลผู้ป่วยวิกฤตของโรงพยาบาลท่าสองยาง จังหวัดตาก ระหว่างวันที่ 1 มกราคม – 31 ธันวาคม 2568 จำนวน 112 ราย แบ่งเป็นกลุ่มภายใน 24 ชั่วโมง จำนวน 77 ราย และภายหลัง 24 ชั่วโมง จำนวน 35 ราย มีการคัดเลือกกลุ่มตัวอย่างแบบเจาะจง (Purposive sampling) วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติเชิงอนุมาน ได้แก่ Logistic regression, Independent t-test, Chi-square test และ Fisher’s exact test รวมทั้งรายงาน Odds Ratio (OR) มีระยะเวลาดำเนินการตั้งแต่เดือนเมษายน - มิถุนายน 2569
ผลการศึกษาพบภาวะเลือดออกในทางเดินอาหารของผู้ป่วยที่ได้รับอาหารทางสายยางภายใน 24 ชั่วโมง และภายหลัง 24 ชั่วโมง ที่ร้อยละ 3.90 และ 8.57 ตามลำดับ โดยไม่แตกต่างอย่างมีนัยสำคัญทางสถิติ (RR=0.45, 95% CI=0.10–2.14, p=0.37) ส่วนภาวะการย่อยอาหารผิดปกติพบร้อยละ 16.88 และ 34.29 ตามลำดับ (RR=0.49, 95% CI=0.25–0.97, p=0.04) และหลังควบคุมปัจจัยร่วม การได้รับอาหารภายใน 24 ชั่วโมง สัมพันธ์กับโอกาสเกิดภาวะการย่อยอาหารผิดปกติที่ลดลง (Adjusted OR=0.36, 95% CI=0.13–0.97, p=0.04) สรุปได้ว่า การให้อาหารภายใน 24 ชั่วโมงมีความสัมพันธ์กับภาวะการย่อยอาหารผิดปกติที่ลดลง ในขณะที่ไม่พบหลักฐานว่าอัตราการเกิดภาวะเลือดออกในทางเดินอาหารสูงขึ้นในกลุ่มที่ได้รับอาหารภายใน 24 ชั่วโมง อย่างไรก็ตาม ผลการศึกษานี้สามารถนำไปใช้เป็นข้อมูลเบื้องต้นในการประกอบการพิจารณาเริ่มให้อาหารทางสายยางระยะแรกในผู้ป่วยที่ไม่มีข้อห้ามทางคลินิกได้
