The Effectiveness of Pharmaceutical Care in Patients Initiating Risperidone Treatment at Rajanukul Institute

Abstract

Objectives To examine the effectiveness of pharmaceutical care in resolving drug-related problems in patients initiating risperidone treatment at Rajanukul Institute.

Materials and Methods A retrospective descriptive study was conducted among 211 patients who initiated Risperidone treatment at the Rajanukul Institute between June 2020 and September 2022. Data were collected from the Hospital Information System and patient medical records, including patient characteristics, drug-related problems (DRPs), methods of problem resolution, and outcomes obtained from drug consultation records. DRPs were classified according to the Pharmaceutical Care Network Europe (PCNE) classification system. Medication adherence was assessed using the Brief Adherence Rating Scale (BARs). Data were analyzed using descriptive statistics, and a paired t-test was applied to compare adherence scores before and after pharmaceutical care.

Results From a total of 211 patients, the majority were males (77.25%) with a mean age of 7.24±7.10 years. Predominant diagnoses included Autistic disorder (52.13%) and Attention-deficit hyperactivity disorder (ADHD) (23.70%). The mean initial dose of risperidone was 0.42 ± 0.33 mg/day. A total of 148 drug-related problems (DRPs) were identified, primarily adverse drug reactions (45.95%), non-adherence to medication (29.73%), and prescribing errors (24.32%). Pharmaceutical care interventions, including problem-solving strategies and follow-up, successfully resolved 81.08% of identified DRPs. Among patients with medication non-adherence, the mean BAR score increased significantly from 44.23 ± 26.08 before pharmaceutical care to 97.03 ± 11.33 after pharmaceutical care.

Conclusion Pharmaceutical care that incorporates medication education, consultation, and continuous patient follow-up facilitates the identification and resolution of drug-related problems and promotes appropriate medication use among patients initiating risperidone therapy, empowers pharmacists to deliver systematic pharmaceutical care, thereby addressing a broader spectrum of issues encountered in patient care.
