Nursing Care for Patients with Relapsed Schizophrenia Associated with Substance Use: A Comparative Case Study

Abstract
Schizophrenia with comorbid substance use is a major factor contributing to relapse. This study aimed to analyze associated factors, nursing problems, and care outcomes using a comparative case study design in two patients at Sriratana Hospital. Participants were selected using purposive sampling. Data were collected from retrospective medical records in 2025, in-depth interviews, and health assessments conducted between March 17 and April 30, 2026. Data were analyzed using content analysis based on Gordon’s Functional Health Patterns.
The findings revealed that both patients shared common factors, including lack of insight, poor medication adherence, and repeated substance use, which were associated with relapse. Patients who received adequate family support and continuous follow-up demonstrated significant reductions in psychiatric symptoms and relapse rates.
The study led to the development of the “Sriratana Relapse Prevention Guideline” and highlights that holistic nursing care combined with continuous family and community support plays a crucial role in preventing relapse.
