Development of a Health Literacy Enhancement Program for Village Health Volunteers as Doctor No. 1 at Ban Wao Health Station, Lao Kwang Sub-district, Non Khun District, Sisaket Province.

Abstract
This Research and Development study aimed to develop and examine the effectiveness of a health literacy enhancement program for Village Health Volunteers (VHVs) as Doctor No. 1, using Borg and Gall's (1983) framework across four phases. Phase 1 samples comprised 103 VHVs; Phase 2 involved 5 experts; and Phase 3 included all 29 VHVs at Ban Wao Health Station. Data were analyzed using descriptive statistics and Paired t-test.
Results revealed that in Phase 1, when assessed against Sørensen et al.'s (2012) four-dimensional health literacy framework encompassing access, understanding, appraisal, and application of health information, VHVs demonstrated a moderate overall health literacy level ( = 3.09), with the application dimension scoring lowest ( = 2.68), and expressed high development needs ( = 4.24), with Telemedicine being the top priority. In Phase 2, the developed program comprising 8 learning units totaling 40 hours, covering all four health literacy dimensions, was rated highly appropriate by experts ( = 4.56, IOC = 0.91). In Phase 3, VHVs showed significantly higher knowledge, OSCE skills, and confidence scores after the program than before at the .01 significance level (t = 15.62, 19.86, and 14.89, respectively). All VHVs achieved excellent knowledge levels and passed all OSCE stations. Program satisfaction was at the highest level ( = 4.73). The 3-month follow-up revealed a 53.17% increase in service recipients. In Phase 4, the program manual received the highest appropriateness rating ( = 4.69).
The developed program effectively enhanced VHVs' health literacy across all four dimensions and demonstrates strong potential for replication in areas with similar contexts.
