Abstract
This research employed a Sequential Explanatory Mixed Methods Design to study the situation, develop, test, and analyze relationships of a family participation model for diabetes prevention behavior modification among at-risk populations. The study comprised three phases. Phase 1 was a cross-sectional study of 142 at-risk individuals with FBS 100–125 mg/dL, revealing low knowledge (𝑥̅ = 11.81), moderate attitudes (𝑥̅ = 46.02), moderate health behaviors (𝑥̅ = 70.49), and moderate family participation (𝑥̅ = 82.75). Phase 2 was a phenomenological qualitative study involving in-depth interviews with 15 families and two focus groups with 18 participants. Four main themes were identified, leading to the development of the KOK FAMILY Model comprising KOK (Kok Community Context, Ownership of Health, Kinship Network Support) and six FAMILY steps, integrating six theoretical frameworks with S-CVI = 0.92. Phase 3 was a quasi-experimental study with 31 experimental and 31 control families over 12 weeks, showing significant improvements with large effect sizes in all variables: knowledge (difference +4.45, S.D. = 0.85, Cohen's d = 5.24, p < 0.001), attitudes (difference +13.08, S.D. = 2.84, Cohen's d = 4.61, p < 0.001), health behaviors (difference +19.85, S.D. = 3.88, Cohen's d = 5.12, p < 0.001), family participation (difference +24.51, S.D. = 4.82, Cohen's d = 5.08, p < 0.001), and reduced FBS (difference -8.37 mg/dL, S.D. = 2.90, Cohen's d = 2.88, p < 0.001). All variables demonstrated large effect sizes (Cohen's d > 0.80), indicating strong clinical significance. The experimental group outperformed the control group significantly in all key variables (p < 0.001). Family participation correlated positively with behaviors (r = 0.718, p < 0.001), with instrumental support as the strongest predictor (β = 0.468, R² = 0.724, p < 0.001).
These findings indicate that the KOK FAMILY Model demonstrates both statistical and clinical effectiveness, with potential for implementation within primary health care systems. Health Promoting Hospitals and District Public Health Offices in rural communities of Northeastern Thailand with similar sociocultural contexts may integrate this model into existing diabetes risk management services by redefining the role of Village Health Volunteers as family health supporters and utilizing communication technology as a mechanism for ongoing health monitoring, thereby contributing to sustainable community-level diabetes prevention.
