Incidence of neonatal hearing loss by otoacoustic emission screening at Aranyaprathet hospital

Abstract
Newborn hearing screening enables early detection of hearing abnormalities. This study aimed to determine the incidence of auditory brainstem response (ABR)-confirmed hearing loss, describe otoacoustic emissions (OAE) screening outcomes, and identify factors associated with abnormal OAE results among newborns at Aranyaprathet Hospital. A cross-sectional analytic study using retrospective data was conducted among all live-born infants delivered between June 1, 2023, and May 31, 2024. A total of 1,294 newborns were included. Descriptive and inferential statistics were used, with statistical significance set at p < 0.05.
The first OAE screening showed a recall rate of 14.3%. After repeat screening, 35 infants were referred for ABR testing, representing 2.7% of all newborns, and 31 underwent ABR assessment. ABR-confirmed hearing loss was identified in 2 infants, corresponding to 0.15% or 1.55 per 1,000 live births. Overall, 1,258 infants had normal OAE results, and 36 had abnormal OAE results. The most common risk factors were low birth weight, prematurity, congenital infection, and aminoglycoside exposure for more than 5 days. Congenital craniofacial abnormalities were associated with abnormal OAE results in univariable analysis (p = 0.028), whereas multivariable logistic regression did not identify any statistically significant associated factors. Factors associated with abnormal OAE results should be interpreted cautiously because the numbers of infants with abnormal OAE results and confirmed hearing loss were small.
