Diabetes Care System Development for Blood Sugar Control via Carbohydrate Counting at Nong Wa Sub-district Health Promoting Hospital, Benchalak District, Sisaket Province

Abstract

This study employed a Research and Development (R&D) framework across three phases: Phase 1 examined the situation using mixed methods, Phase 2 developed the service system through Participatory Action Research (PAR), and Phase 3 evaluated outcomes using a one-group pretest-posttest quasi-experimental design. The study aimed to develop a diabetes care service system for blood glucose control through carbohydrate counting at Nong Wa Sub-district Health Promoting Hospital, Benchalak District, Sisaket Province. A total of 60 patients with type 2 diabetes mellitus were enrolled from August 2025 to July 2026. Data were collected using a carbohydrate counting knowledge test, a self-care behavior assessment, and a satisfaction questionnaire. The analysis used descriptive statistics and a paired samples t-test.


Baseline findings revealed that only 27.22% of patients achieved HbA1c targets, falling below the Ministry of Public Health benchmark of 40.00%, and all participants demonstrated low carbohydrate counting knowledge. The developed service system comprised four components: eight group education sessions, individual counseling, biweekly telephone follow-up, and local Northeastern Thai cuisine learning materials. Post-intervention, significant improvements were observed across all outcomes (p < 0.001): knowledge scores increased from 17.32 to 37.02, HbA1c declined from 6.95% to 6.12%, FBS from 144.95 to 119.85 mg/dL, 2 hr-PPG from 198.23 to 163.98 mg/dL, and self-care behavior scores from 60.25 to 66.75. Overall patient satisfaction was rated at the highest level (  = 4.70 out of 5.00). The diabetes care service system developed through carbohydrate counting, grounded in the Isan food cultural context and community participatory processes, significantly improved knowledge, self-care behaviors, and blood glucose control in the studied sample. However, long-term follow-up is necessary to confirm the sustainability of these findings.
