บทคัดย่อ
การวิจัยนี้มีวัตถุประสงค์เพื่อพัฒนารูปแบบการดูแลผู้ป่วยจิตเวชและยาเสพติดในชุมชน อำเภอขุนหาญ จังหวัดศรีสะเกษ ที่เหมาะสมกับบริบทพื้นที่ ดำเนินการวิจัยและพัฒนา 3 ระยะ ได้แก่ ระยะที่ 1 การวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง กลุ่มตัวอย่าง 385 คน ระยะที่ 2 การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม กลุ่มตัวอย่าง 60 คน และระยะที่ 3 การวิจัยกึ่งทดลองแบบ One Group Pretest-Posttest Design กลุ่มตัวอย่าง 90 คน ครอบคลุมพื้นที่ 12 ตำบล ระยะเวลา 9 เดือน
ผลการวิจัยพบว่า ปัจจัยที่มีความสัมพันธ์สูงสุดต่อประสิทธิผลการดูแลผู้ป่วย ได้แก่ การสนับสนุนจากผู้บริหาร (r = 0.742) ความรู้และทักษะของบุคลากร (r = 0.724) และการมีส่วนร่วมของชุมชน (r = 0.698) รูปแบบการดูแล
ที่พัฒนาขึ้น คือ "โมเดลขุนหาญ" (Khun Han Model) ประกอบด้วย 4 องค์ประกอบหลัก ได้แก่ 1) การคัดกรองและประเมินผู้ป่วยในชุมชน 2) การพัฒนาศักยภาพบุคลากรและอาสาสมัครสาธารณสุข 3) การให้บริการและติดตามอย่างต่อเนื่อง และ 4) การสร้างเครือข่ายและการมีส่วนร่วมของชุมชน
ผลการประเมินประสิทธิผล พบว่า หลังการใช้โมเดลขุนหาญ การเข้าถึงบริการของผู้ป่วยเพิ่มขึ้นสูงสุด
(ร้อยละ 61.65) ความพึงพอใจโดยรวมเพิ่มขึ้นร้อยละ 56.89 และประสิทธิผลโดยรวมเพิ่มขึ้นร้อยละ 80.51 โดยทุกตัวชี้วัดมีนัยสำคัญทางสถิติ (p < .001) และมีขนาดผลกระทบอยู่ในระดับสูงมาก (Cohen's d > 1.60) โมเดลขุนหาญมีความเหมาะสมกับบริบทชนบทและสามารถขยายผลสู่พื้นที่อื่นที่มีลักษณะคล้ายคลึงกันได้
