Outcomes of Nursing Management in Patients' Preparation for Colonoscopy through a One-day Surgery Outpatient Service

Abstract

Background: The success of colonoscopy examination largely depends on adequate visualization of the colonic walls, which in turn relies on the preparation of the colon. Vachira Phuket Hospital has developed a nursing management protocol for preparing patients undergoing colonoscopy examinations through a one-day surgery service since 2019. However, there is still a lack of systematic data collection and clear reporting of outcomes of care.

Objective: To retrospectively investigate the outcomes of nursing management in preparing patients for colonoscopy examinations through the one-day surgery service at Vachira Phuket Hospital over three years, from fiscal years 2021 to 2023.

Method: This study utilized a retrospective observational study design. Outcome data were collected using data collected forms validated by three experts. Before actual data, two researchers independently collected five pilot cases to enhance interrater reliability. Data analysis was conducted using descriptive statistics.

Result: During fiscal years 2021-2023, 70, 131, and 158 patients underwent colonoscopy examinations in the one-day surgery service, respectively. The majority achieved excellent colon cleanliness, with percentages of 36.5, 58.4, and 60.81, respectively. The average duration of receiving the service from entering the endoscopy room to returning home was 427.27+68.70, 344.91+64.52, and 363.66+74.33 minutes The colonoscopy success rates were predominantly 95.24, 96, and 97.90 percent, respectively. The main reason for the unsuccessful colonoscopy was the presence of a large mass obstructing the lumen, causing an inability to pass the endoscope.

Conclusion: Nursing management in preparing patients for colonoscopy examinations in the one-day surgery service contributes to good results in colon cleanliness to successful colonoscopy procedures.
