The Effects of Perception Program of Pulmonary Tuberculosis Prevention in Type II Biabetic Patient at Takuathung Hospital, Phangnga Province

Abstract

Background: Tuberculosis (TB) remains a major public health problem, particularly among patients with type 2 diabetes mellitus (T2DM), who are at increased risk of infection and adverse health outcomes. Enhancing health perceptions and preventive behaviors is therefore essential to reduce such risks.

Objective: To develop and evaluate the effectiveness of a perception-based intervention program for tuberculosis prevention by comparing knowledge, health beliefs based on the Health Belief Model (HBM), and preventive behaviors among patients with type 2 diabetes mellitus before and after participation in the program.

Methods: A quasi-experimental study with a one-group pretest–posttest design was conducted among 39 patients with type 2 diabetes mellitus. The perception-based intervention program was developed based on the Health Belief Model. Data were collected using validated questionnaires. Descriptive statistics and paired samples t-test were used for data analysis.

Results: Following participation in the program, the participants demonstrated significantly higher mean scores in tuberculosis (TB) knowledge, Health Belief Model (HBM)-based perceptions, and TB preventive behaviors compared to the pre-intervention baseline (all p < .001). Notably, TB-related knowledge exhibited the most pronounced improvement, with the mean score escalating from 5.01 ± 1.82 at baseline to 9.32 ± 0.78 post-intervention (Mean Difference [MD] = 4.31). Regarding HBM constructs, perceived susceptibility to TB showed the largest gain (MD = 0.71), followed by perceived severity (MD = 0.66) and perceived benefits of preventive practices (MD = 0.64). Furthermore, TB preventive behaviors improved significantly, with the mean score rising from 2.14 ± 0.45 to 2.84 ± 0.30 (MD = 0.70).

Conclusion: The perception-building program proved highly effective in enhancing knowledge, perception, and preventive behaviors regarding tuberculosis among type 2 diabetic patients. It serves as a viable intervention for promoting tuberculosis prevention among at-risk populations.
