Developing a Telemedicine Model for Integrated and Participatory Access to Healthcare Services for Children with Special Needs through Health Networks and Pai District Communities, Mae Hong Son Province

Introduction: Children with special needs in remote areas still face limitations in accessing specialized healthcare services due to geographical, personnel, and resource constraints. The application of telemedicine, combined with the participation of health networks and communities, is therefore a crucial approach to reducing disparities and increasing access to quality services.
Objectives: To study the situation of telemedicine utilization by children with special needs, develop an integrated remote healthcare service model, and evaluate the effectiveness of the developed model in Pai District, Mae Hong Son Province.
Methods: This research employed a Research and Development (R&D) approach combined with Participatory Action Research (PAR). The sample consisted of 100 children with special needs and their parents, and 20 key informants. Research instruments included questionnaires, in-depth interviews, and focus group discussions. Data were analyzed using descriptive statistics and content analysis.
Results: The number of telemedicine service recipients showed an increasing trend. The most common disorder treated was Attention Deficit Hyperactivity Disorder (ADHD), followed by depression and autism. The developed model is an integrated service system linking hospitals, primary care units, educational institutions, families, communities, and local administrative organizations through six key processes: collaborative policy formulation, network building, and technology system development. Multidisciplinary care, capacity building for personnel and community, and continuous monitoring and evaluation resulted in an overall high level of effectiveness (Mean = 4.36, S.D. = 0.56).
Conclusion and Discussion: The developed Telemedicine model can increase access to healthcare services for children with special needs, reduce limitations of distance and travel costs, promote collaboration among health networks and communities, and support continuous care for children with special needs. It is suitable for expansion into rural and remote areas with limited access to healthcare services.
